La pianificazione della dimissione e il Nucleo di Continuità Ospedale-Territorio

Riassunto: Introduzione - La pianificazione organizzata delle dimissioni protette è il sistema multi-professionale per facilitare un’erogazione continua, prevedibile e sicura dell’assistenza verso pazienti, familiari e caregiver in tempi di attesa accettabili. Questo studio esamina l’efficacia del Nucleo Centrale di Continuità Ospedale-Territorio (NuCOT) in risposta alle dimissioni protette.
Metodi - Studio osservazionale retrospettivo (dicembre 2017 - settembre 2018), campione non probabilistico di 1.299 schede riferite a pazienti adulti. Dati raccolti in forma anonima dal database di un’Azienda Sanitaria del nord Italia.
Risultati - Il 72,4% del campione risulta avere una età >70 anni, le U.O. che richiedono l’intervento del NuCOT propongono per i propri pazienti la Lungodegenza per il 42,9%, le strutture ad Alta Attività Assistenziale o CRA per il 41,4% e le Post Acuzie (15,2%). Le richieste convalidate dal Nucleo di Continuità sono state il 94,3%. Il 90,8% dei pazienti/caregiver hanno accettato il percorso. Gennaio è il mese in cui le U.O. hanno avuto un tempestivo riscontro dal NuCOT (0,13± 0,45), dicembre è stato il mese in cui i trasferimenti sono stati celeri (1,25±1,06). Nel periodo primaverile ed estivo sono state registrate contratture sui tempi.
Conclusioni - I tempi dalla richiesta delle U.O., alla convalida del Nucleo e al trasferimento del paziente, si sono dimostrate in linea con la letteratura. Il NuCOT risponde tempestivamente ai bisogni di assistenza personalizzata mettendo al centro il paziente nei percorsi di cura.

Abstract: Introduction - Organized planning of protected discharge is the multi-professional system to facilitate the continuous, predictable and safe delivery of assistance to patients, family members and caregivers in acceptable waiting times. This study examines the effectiveness of the Central Continuity Hospital-Territory (NuCOT) in response to sheltered discharges.
Methods - Retrospective observational study (December 2017 - September 2018), a non-probabilistic sample of 1.299 cards referring to adult patients. Data collected anonymously from the database of a Health Agency of northern Italy.
Results - 72.4% of the sample were aged >70, the U.O. that require the intervention of the NuCOT propose for their patients the long-term care for 42.9%, the structures with High Assistance Activity or CRA for 41.4% and the Post Acuzie (15.2%). The requests validated by the Continuity Unit were 94.3%. 90.8% of patients/caregivers accepted the course. January is the month in which the U.O. they received a timely response from the NuCOT (0.13 ± 0.45), December was the month in which the transfers were quick (1.25 ± 1.06). In the spring and summer period, contractures were recorded over time.
Conclusions - The times from the request of the U.O. to the validation of the Nucleus and to the transfer of the patient have proved to be in line with the literature. The NuCOT responds promptly to the need for personalized assistance by focusing on the patient in the care pathways.
